Nursing reports

I am an Lpn in a LTC facility. We currently use what's called a huddle binder to put issues or important info on pts to "huddle" with our aides. Well I don't like it. I have used "24 hour" reports at other facilities and they worked well. Plus, where I am now is not organized and things are being missed and not passed to the new shift. My question is, what do you use at your LTC facility? And do you have a better way to communicate from shift to shift. Thanks!!
