nursing interventions are treatments, things a nurse will do, for some specific thing that is going on that the patient needs help with. a stroke is a medical problem. decreased cellular exchange describes what is going on in the person's body physiologically. that's not much practical help to us as nurses because we help people accomplish their adls (activities of daily living). so, you need to describe (have evidence of) this decreased cellular exchange due to a stroke.
