I am trying to find a nursing diagnosis for a care plan that involves terminating a nurse-client relationship. The interventions would involve pointing client to wellness/PHNs in surrounding area, but cannot find a NANDA diagnosis. Does anyone know what it might be called?

Off the top of my head, I'd say you terminate the nurse-client relationship when the goals have been met and/or the client wants to end it.
The best way I found to determine the nursing diagnosis was to ask myself, what do I feel I need to do for this client?
Then I looked at what problem I would be easing or relieving.
That told me where to look for my NANDA diagnosis.
(If it is any comfort, I've been in the field now for a year, and I've yet to see a nursing diagnosis used in a hospital or clinic.....)
