I am working on my care plan and I am having trouble coming up with nursing dx's. My pt. is an 83 yr. old female admitted with the dx of syncope. Past medical hx of HTN, Hyperlipedeima, alzheimers, dizzy spells and vit D deficiency. According to her dr. notes she had experienced unexplained syncope since childhood and there are no test results to go on other than labs. Hgb, RBC, Hct, protein, albumin and vit D all are low. Obviously I can use risk for falls and I am considering decreased cardiac output..........any other suggestions? Thanks!!
