PLEASE HELP "cries face"
I haven't written an assignment in a longggg time and I just want to know if i am on the right track please someone help me out!
I have added the case study and assessment task below which I have picked.
I have addressed that hypertension and hyperlipidemia are my 2 priority nursing problems. Is this correct?
Then hypertension relates to this patients assessment data because of her BMI and not taking her meds blah balh (evidence)
Would nursing interventions be; getting a dietition involved and educating the patient on importance of taking meds?
CASE STUDY
Beryl is a 50 year old woman with Hypertension who is attending a Healthy Heart Workshop at her local Community Health Centre. Beryl's blood pressure is 160/95, she is 150cms in height and her weight is 105kg. Beryl reports that she drinks two to three glasses of wine per night, eats a lot of take away foods as she's too busy with her executive management job to think about cooking. She is a single mother caring for three school aged children to always cook. She states that she rarely exercises due to lack of time. Beryl visited her GP last week after feeling unwell. Pathology was taken and Beryl's GP report shows that her total cholesterol is 6 mmol/L, Low Density Lipoprotein 3.1 mmol/L and High Density Lipoprotein 0.8mmol/L. The GP commenced Beryl on medications but she states "I keep forgetting to take them, my life is just so stressful at the moment. I really don't have time to be here, I need to be at work".
ASSESSMENT TASK
Using the clinical reasoning cycle you are required to assess, plan, implement and evaluate your care for the patient. After identifying the relevant assessment data you must identify two (2) priority problems for your patient. For each priority problem identified, list two (2) nursing interventions (collaborative/independent) linking to your research of the literature. Include a one to two line rationale for each intervention. Evaluation of your care is essential
Introduction (150 words approx): State an overview of the patient's condition and identify the TWO priority nursing problems. Give a brief plan for the essay topics.
For each of your identified priority problems (950 words approx): Briefly explain how it relates to the patient's assessment data. You will need to draw on your understanding of pathophysiology to explain the link between the problem and the patient's signs and symptoms. Identify TWO priority nursing interventions to address the problem. Justify the nursing interventions you will implement (explain what is involved in implementing the intervention, why it is suitable, any relevant positive or negative aspects/considerations) and support/justify the use of these interventions with reference to current evidence-based literature. Outline the evaluation criteria/data that would indicate that each intervention is improving or resolving the identified problem.
Conclusion (150 words approx): Briefly restate the patient's condition and problems and how the nursing interventions can benefit the patient's health

Hypertension and hyoerlipidemia are medical diagnoses, not nursing diagnoses. Do you have a nursing care plan book to assist you with the assignment? Nursing diagnoses are based on the nursing assessment of the patient, not the medical diagnoses.

No I don't unfortunately I have to access everything online.
Okay so would a nursing diagnosis be more like: Non-compliance with healthy lifestyle behaviours related to family/pressures/stressors as evidenced by self report of smoking, diet and BMI in obese range.

In the case, 90% of the problem situations discussed are r/t tress, diet and overall lifestyle management.
HTN and lipids are not the "problems". They are aiming at the management of these problems. Re-read it and look at all the described behaviorial factors that contribute to this problem. Its all "modifiable risk factors".
If you go straight for the medication management which is briefly mentioned as the last sentence, you will have missed the aim they are looking for.
