Nurse Assignment Sheet

I work on a 31 bed med/surg floor and am curious as to what everyone's Nurse Assignment sheet looks like. Ours has the patients room numbers down the side then in the middle has team 1, team 2 etc etc. Each team lists the nurse and aid or LPN and the list of room numbers they are assigned. Anyone have a copy of what their facility uses to compare? We are looking for a new sheet format.

Ours are pretty simple.The date and shift are at the top and the nurses names are listed down the side with room numbers after them.
