I can't answer the first two, never worked max
If you're only asking about max can't answer 3 either but in all likely hood on all levels an officer has to be there

In a max prison a CO has to be present with us to go to the door for s/c and med pass. If we enter a room an entire crew must be a be present 4 CO and a LT. Noone can enter a cellfor an reason unless the crew is present. This prevents an I/M from luring a staff member into the cell for bad purposes.
