NPI

How do I apply for an NPI # when I am a travel nurse.  I have a compact RN license (primary state is from West Virginia).  I am a travel nurse and took my AANP Boards in CO.  Colorado is also where I am currently a travel nurse in.  I am hoping to get a job as a FNP in Colorado.   Which address should I put down in the NPI application?

If you are “traveling”, I would use the address of the company for which I work. If you find that you bounce around between companies, it might be prudent to set up a personal llc or something similar that utilizes an address that’s not your home address.

Do not set up you’re home address. I made that mistake and for at least two years his my home address publicly available despite working an hour away.

funny story on that too.  I literally had a pharmaceutical rep driving through my neighborhood trying to find my practice when one of their colleagues visiting our practice caught my name and mentioned that. They found my info online thinking I had a new clinic in a different district.
