  1. How do I apply for an NPI # when I am a travel nurse. I have a compact RN license (primary state is from West Virginia). I am a travel nurse and took my AANP Boards in CO. Colorado is also where I am currently a travel nurse in. I am hoping to get a job as a FNP in Colorado. Which address should I put down in the NPI application?
  2. One of my professors asked me if it was always reversible and I always thought it was. She told me to look it up. From what I have found in most cases it is reversible but National Institutes of Health states, delirium is not always reversible and can result in long-term cognitive impairment. I was just curious as to how it is not reversible. A lot of articles and journals say that delirium is reversible most of the time but do not state when it does not become reversible. I am just curious if anybody else had knowledge of delirium not being reversible.

  5. Hi there, I am having trouble writing out my care plan. My pt is having left sided lower flank pain ( maybe nephritis) Here are my diagnosis. Which do you think should be my top 3? 1. Sleep deprivation related to someone coming in early in the morning 2. Disturbed sleep pattern related to someone coming in early in the morning 3. Fatigue related to someone coming in early in the morning, lack of sleep, 4. Risk for injury related to lack of sleep, not having glasses with her, high RBC 5. Disturbed Visual Sensory related to not having glasses 6. Risk for falls related to lack of sleep, disturbed sleep patter, acute pain, not having glasses 7. Risk for infection related to UTI, past kidney infections 8. Impaired gas exchange related to high level of RBC 9. Risk for activity intolerance related to acute pain 10. Acute pain related to nephritis, inflammation, and infection 12. Impaired Urinary Elimination related to dysuria 13. Risk for fluid imbalance related to dysuria
