I thought I was getting the hang of care plans, but this semester I have started to struggle again. I did a care plan for mother and baby and was told to look for another NANDA-Dx for the baby. I had done "Imbalanced nutrition: less than body requirements" The baby was less than 12 hours old, and I understand that it's not typical for an infant to breastfeed at this time, but I thought it would be the best fit.
The baby had a LATCH score 4/10
the mother attempted to breast feed 3 times and the 3 times were unsuccessful and the infant kept falling asleep.
Overall everything appeared to be WNL.
I thought of switching my dx to ineffective breastfeeding aeb LATCH score 4/10
any suggestions?

I wouldn't do the first one, I'd go with the second one. Newborns don't need a lot the first few days.
