I was told by a pa anything that happens in the ed we can fix. If it's low, we make it go higher.
I accidentally gave a pt 10 u of insulin when the order was for 2u.
The way the computer was set up, it looked like 10u but that was part of a scale.
We no longer use the sliding scale. Just specific units to be given.
He was watched and fed and no issues. His bs was just below 200 when he was given it.

Had a sitch that reminded me that ANYTHING IS POSSIBLE, do your assessment. Briefly; Pt with Blood Glucose 540ish, also, Hypertensive to the effect 215ish/ 125ish consistently. Previous 3 checks at 1/2 intervals. We gave insulin, came down to 390ish, Doc and I consulted on next insulin dosage, and HTN treatment. 2 more Units insulin SQ and 0.1 Catapres. I walked into the room, no change is appearance of patient, no s/s of distress etc. I. Gave the Catapres and BEFORE it even entered pt's stomach I checked the B/P....132/78!!!!!! Who wouldathunk??? Not me....no hurt, watched closely and gave another 1000ml NS. That was a little disturbing, ever happen to you, where no one got hurt, but you got a wake up call?
