I am doing a speech on communication in nursing for my communications class. I would like to open with a gripping story about how a lack of good communication with a patient led to an avoidable bad outcome.
I have been searching online but have not found any news articles about such an event.
Can anyone think of any examples that may have been published somewhere?
It would be a big help :)

I have limited access at home, but suggest you try "sentinel event" , "case study", and or "safe hand-off" as search terms.

How about errors related to d/c instructions, med administration errors, allergies, etc that happened as a result of using family members as ad-hoc interpreters? http://www.medscape.org/viewarticle/534045_2
