Need published example of bad nurse patient communication causing tragety

I am doing a speech on communication in nursing for my communications class. I would like to open with a gripping story about how a lack of good communication with a patient led to an avoidable bad outcome.

I have been searching online but have not found any news articles about such an event.

Can anyone think of any examples that may have been published somewhere?

It would be a big help :)
