I'm in my OB clinical rotation. We are working on Careplans that have to be written before we actually see a c-section or lady partsl delivery. Its a working care plan. I need help with some nursing diagnosis' for the patient while actually in the labor process, not after but during labor. We need 3 diagnosis for a lady partsl and 3 for a c-section. We have to prioritize the diagnosis. SO ANY SUGGESTIONS? Thank you.

Your text book is a great place to look but I'm sure you must have already looked there. Well having had 3 children and being a nurse the first thing that comes to mind is pain, then there is risk for bleeding and risk for altered resp status. Hope thats enough to get you started in the right direction. Good luck

since i am sure this is not your first care plan, you must be aware that a care plan is about the problems the patient has. even if you didn't read your ob textbook what is abnormal about what is happening to a lady having a baby? pain is not normal. it is happening because of the muscle contractions of the uterus. so, acute pain is one diagnosis. do you suppose the patient might have some anxiety about the whole birthing process? do you think she may have any learning needs? what about risk factors such as infection, injury or fluid losses?
do you have a taber's cyclopedic medical dictionary? in the appendix at the back of the book are nursing diagnoses grouped by diseases and disorders. my copy from 1997 (the 18th edition) lists 4 diagnoses for the first stage of labor (the active stage).
