i am certified nurse. I was taking care one pt and while I was going through pt previous day charting, I accidentally altered & filed charting on my name and today date. So what should I do now?

Actually I was going through pt previous day charting. I accidentally altered and filed it on my name and today date on computer. It was charted 2 days ago by someone but now it shows my name and recorded today. Should I write a comment in box stating that I accidentally filed? Please help.thank you.

Unchart it, and write a comment saying why. It sounds like you didn't have the pt on that day and you accidentally charted, right?

Yes. I was going through or behavior but I accidentally did something wrong and filed it.

When I checked the charting it shows I have taken two days ago and recorded today. Because I touched it but I didn't changed anything.
