need help regarding CES

Hi everyone!

I'm a nurse from the Philippines and I'm about to start filling up my CES application form. I've encountered some confusing items on the form. Here they are:

Item 2:

*CGFNS/ICHP identification number

(i don't have this yet. will i just write N/A on the box provided?)

Item 9:

*citizenship ID number

(i don't know what to write on this item.. i don't think i have this. can

i just place N/A on this item?)

Item 12:

*name of diploma or certificate in its original language

(what exactly do i have to write on this item?)

*degree obtained

(do i have to check this item for the primary, intermediate, and

secondary schools attended?)

Item 14:

*report recipients

(I'm going to have to send a copy to one recipient only- NM BON.

So ,can i fill up the first recipient only and just leave the second

recipient blank? or i have to write N/A on it?)

*if i'm choosing to send a copy to a board of nursing, whose name will

i have to write on the box provided for the name of contact person

or title?

CES report has to go to the BON direct from CGFNS so if you put down the state as a recipient then they should receive their copy. The copy you have is just your copy

If you have no CGFNS ID number then leave either blank or N/A

If you have no US ID number then again leave blank or N/A

What was the title of your course? That is what you write. If you attended school less than 10 years ago then you have to include details of schools and any education

If you only want the report to go to one BON then just put the one BON down. If you can find a contact name of the state website use that otherwise leave blank (I can't remember putting any contact names down for my report and it got to the BON OK plus I am sure CGFNS will have contact details for all BONS that they send reports to)
