Need help with my care plan

Hi guys

I'm supposed to do write a care plan starting with the patho but the problem is I am stuck on the patho. My client had a hemorrhagic stroke and they came into the hospital with alter mental status and lethargic as the chief medical complaint. My client has high blood pressure and type 2 diabetes mellitus and abnormal renal function. What would I write as the patho...would it be the hemorrhagic stroke he suffered from? And how would I go about writing it.

Stroke is a medical diagnosis. Care plans are based on your nursing assessment and what the patient needs are. What are your major concerns regarding this patient following yours assessment? Altered loc? Weakness? Cognitive? Not able to maintain a clear airway? Etc.
