I have to do a nursing care plan for a patient with ARF, and I'm not sure what I should use as the most important, "primary", nursing diagnosis! I have some in mind, but I get lost when it comes to the r/t and AEB parts! Help is greatly appreciated!!

Can you be more specific? Do you mean acute resp. failure or acute renal failure?

We are happy to help with homework but need more information. What do you have so far? What semester are you? Is this a real patient? What is your assessment?

Nursing diagnoses come from nursing assessments, NOT medical diagnoses. Common misconception, but there it is. So... what did you assess with your patient, and what did you think was his worst problem when you looked at him and his medical record? Why do you think it was his worst problem? There's your answer, really.

We all have problems with care plan but if we know the presenting signs and symptoms it would help in finding a solution.
