could any one help me formulate a nsg dx?

my patient is an elderly female,77 y.o, dx with dm2. she was admitted due to unresponsiveness but has a rls/gcs ratio of 1/15

on my duty. she has minimal activity because she needs assistance to do her ADL even when getting up on the bed. she has high HGT result of 183 mg/dl.she has PR of 60 and a CR of 62. what nsg dx could i make from this?

there's some information that you posted that i cannot understand. it is because you used abbreviations i do not recognize.

  • what is a "rls/gcs ratio"?
  • what is a "hgt"?
  • what is a "cr" ?

also,

  • you need to be more specific than just saying minimal activity or needs assistance to do her adls. getting up on the bed is mobility. describe what assistance is needed. a nursing problem (nursing diagnosis) is about what the patient cannot do and it must be described because goals are written to describe the opposite (the improvement you will observe).
  • you have provided no physical assessment data. was a head-to-toe physical assessment done? lung sounds? abdominal sounds? any edema? if gcs is a glasgow coma scale what are the specific results (not the number)? orientation, hand grips, push pulls?
