    could any one help me formulate a nsg dx? my patient is an elderly female,77 y.o, dx with dm2. she was admitted due to unresponsiveness but has a rls/gcs ratio of 1/15 on my duty. she has minimal activity because she needs assistance to do her ADL even when getting up on the bed. she has high HGT result of 183 mg/dl.she has PR of 60 and a CR of 62. what nsg dx could i make from this?
