Multiple-strength Solution

Published

I am having with difficulties determining which diluent to use in Multiple strength reconstitution.

Here is a problem 1) Order: Penicillin G potassim 400,000 U IM q.i.d.

The Supply is a vial containing 1,000,000 units of powdwered penicillin G potassium with the following instructions: Amount of diluent to be ADDED: Approximate Concentation

9.6 ml 100,000U/ ml

4.6 ml 200,000 U/ml

1.6 ml 500,000 U/ ml I know how to each answer for the concentration. I am just not sure about which concentration do you use for an IM injection.

2)Enteral feedings= 2/3 strength Ensure 6oz via gastromy tube over 2 hour supply 4,8,12 oz can. Note which supply would require the least discard of unused formula. I am confused on how to set up the problemfor enteral feeding

Thanks do Much,

1) order: penicillin g potassim 400,000 u im q.i.d.

the supply is a vial containing 1,000,000 units of powdwered penicillin g potassium with the following instructions: amount of diluent to be added: approximate concentation

9.6 ml 100,000u/ ml

4.6 ml 200,000 u/ml

1.6 ml 500,000 u/ ml

you are going to inject this into somebody's butt, so you want to use the least amount of solute as possible. use the
1.6 ml 500,000 u/ ml.
you would give

400,000 units
(dose desired)
/500,000
(dose on hand)
x 1.6 ml
(amount it comes in)
= 1.28 ml,
rounded up to
1.3 ml
(amount you actually inject)

2)enteral feedings= 2/3 strength ensure 6oz via gastrostomy tube over 2 hour supply 4,8,12 oz can. note which supply would require the least discard of unused formula. i am confused on how to set up the problem for enteral feeding,

a 6 ounce 2/3 strength ensure will be

  • 4 ounces of ensure + 2 ounces of water (2/3 x 6 oz = 4 oz)

now double that since it needs to be given over 2 hours. you will need 8 ounces of ensure and 4 ounces of water. you will need an
8-ounce can of ensure
.

Thanks Daytonite,

I greatly appreciate your help with these problems. This clears everything up for me

thank You,
