• Official completion documents from the Illinois monitoring program showing discharge in good standing.
• A written statement detailing the original incident, recovery maintenance, and uninterrupted practice history since 2016.
• Official license verifications from all states where a nursing license is or was held.

Disclose the disciplinary action explicitly on the Minnesota application. Failing to disclose board orders visible on Nursys is classified as material misrepresentation under board statutes, which triggers new disciplinary proceedings. Standard endorsement processing takes four to six weeks, but files requiring legal or committee review routinely take 12 to 16 weeks.
