Medicare Question

I am trying to gather some information about the criteria needed in a patients chart for medicare reimbursement. I have not been able to find any articles that list what has to be in the chart for medicare. I need all the help i can get. Thanks for your time. ~K~ from TX

Are you referring to an acute care setting, long term care or what? I do not know if a list exists, other than the "Red Book" which lists ICD-9 codes. However, if your question was a little more specific you might get the answer you need from someone. Are you a nurse? Student? Why do you need the info and how much do you need?

I am a student nurse soing research for a NP at my clinical site. In the office (family practice) and (Internal medicine)the 3 doctors and 2 NP's are all using diffrent progress notes, history forms ect. She is trying to get all of them on board with the same paper work. And is trying to gather the information needed to pursuade them in to agreement. Articles with this kind of information is impossibe to find.
