MDS PPS and LTC Tools Needed

Are there any experienced MDS Coordinators willing to help me?

I'm looking for long term care and medicare mds calendars. I'm also looking for a worksheet or screening tool that is used to assess residents when they enter a SNF for adl indexing and extensive services. Does anyone have a long term care mds tool that calculates all assessment reference dates after you enter the date of admission? In other words, when you enter the admit date it automatically calculates all other assessment reference dates that will be due. I'm interested in all tools related to the mds process. Thank-you

I need a little guidance I went to the web site given and printed out the tool but then another coworker printed out a similiar one but from LTCSbooks.com and both of them are different with a 2 day difference. Sorry but which one is correct?

Keep in mind that PPS scheduling got more complicated recently due to start and end of therapy, and especially change of therapy. If all you need are the main PPS assessments, and you have a separate tool for the OMRAs, then a simpler form may do, but the forms need to be fairly complex to track both PPS and OMRAs together.
