For Those Who Are Mds Coordinators,i'm Having A Problem Figuring Out Where Would You Put On Section M Of The Mds A Dry Scab, No Matter What Size, That Is Over A Pressure Point Area?

1. I asked the mods to move this for you so that you get a better response. There is an actual MDS sub-forum within the main geriatric forum. You will want to post these types of quesions there.
2. As to your actual question, I would want more investigation as to the cause of this "scab". If it is a scratch (think itchy butt/coccyx if you will), I am not going to code this as a pressure ulcer, but as an abrasion. On the other hand, if this is on the heel, I would try to figure out if this is actually pressure or maybe stasis formation.

If it is just a scab, I code it as an abrasion (M4a)...if it is eschar over a pressure area, it is supposed to be coded as a stage IV.
