There are a few different types of programs for patients in the community utilizing assisted medication programs. Staff members trained in medication education can assist patients in setting up their meds and subsequently monitor them for compliance. This type of program is supervised by a psychiatrist and an RN.

One study done in the 1990's showed that that up to 80% of psychiatric readmissions or recidivism can be due to medication noncompliance, so these community med programs can assist patients to remain in the community and maintain a relatively stable status.

Other medication programs involve sample medication programs and patient assisted programs. Basically, a psychiatrist can prescribe a medication and give the patient sample meds until a medication assisted program kicks in. These services are offered through the various drug companies and are made available through the drug reps.

I'm not familiar with the suboxone forms for tracking, but a short Google search yielded some info. This will link you to one such form I found:
