LVN application

guys, pls help me how to download the application form for LVN Licensed Vocational Nurse.., I failed once NCLEX RN & Ive decided to challenge the LVN exam., i am a foreign nurse do i need to send my application in the Philippines which I graduated my nursing course.., to be filled up by the registrar/dean like record of nursing program something like that.. same thing I did with my NCLEX RN.. Pls help im depressed...

You are not challenging the exam you are applying for licensure. Are you in the US with a work visa otherwise would not recommend going for PN as it will not help you with immigration. When applying for PN you have to start all over again and sent all your documents again as usually different departments. Personally I would concentrate on passing RN
