Okay so me and my husband lost everything in fire about 3 months ago and moved to Missouri with my parents!
im looking to get My CNA certification switched over but have not worked in the field for just about two years now! i have ordered the Application for enrollment by reciprocity but i do not know my license number, can anymore give me so guidance as to how to obtain that information. It would be greatly appreciated!

Did you check out your old state's board of nursing website? I know some states keep an online database & you could probably look it up by putting in your name and/or last 4 digits of your SSN.

Your state should have website were you can look. Here in NY , we go the Online Profession Verfication website http://www.op.nysed.gov/opsearches.htm#nme then we choose our profession , then put in last and first name then Viola. It has the #, expiration date and state. I hope this helps.
