I am a 25 year RN recently transitioned into adolescent psychiatric residential nursing(barely a year) and would like any resources on documenation, with emphasis on restraints. I am getting old I guess, and unable to find what I'm looking for online. Please help. Any resources or just advice is very much welcomed.
New to this site as well, and for the life of me, cannot seem to figure it out either. lol

Your facility should have a flow sheet for restraints that has been approved by JACHO and their legal team at the hospital. Unless something out of the norm or beyond what you would be charting on the flow sheet happens, it's supposed to include everything you are supposed to chart.

Can you be more specific - the joint Commission is currently pushing to do away with restraints in all patient populations. Adolescents in a residential facility should be stable enough so as not to require chemical and physical restraints.
