I want to add- this is certainly not something that you will draw up as the School Nurse. There will be a standard form that is specific to your state. Google it or call a local pediatrician office

I have no problem doing it. I have time, most days, for a good project. We only have around 275 kiddos & very few medical concerns.

There doesn't appear to be a standard. High school athletes, yes, but not K kiddos. Heck, my district doesn't even require them but we don't advertise that. :/
