I didn't know I needed a separate license to do IV's.

In order to help you, you'll probably need to give readers more information. Specifically - what state you work in, in what capacity, what license you hold, etc. Then you might find someone familiar with your certification.
Hopefully, you didn't mean that it expires at the end of this week. Eep!
Since I have no idea what you're talking about....I'd say.....If you're currently employed, ask your manager or HR department. Otherwise, asking the body which issued it in the first place is probably your best bet.

Is it a license or certification? Licensure is done by the state. Certifications are awarded after course completions by the provider of the course. In other words, go back to the authority that issued the documentation for the best answer to your question.

If you are certified in IV therapy, look at the materials sent to you on what is required to re-certify or check here: http://www.ins1.org/
If licensed by your state, call your state board or go online to their website for information. Allnurses provides weblinks to all the state board websites under the "Resources" tab above.
