Iv start question

Published

So I just started working in the ER and had a question about starting IVs. I know how to locate them and can usually get a flash when inserting thrm but then I am unable to thread it in and have to pull it out some in order to draw blood and them am able to float it in. (If that makes sense). This happens every time. Anyone have any tips or know what I could be doing wrong?

Make sure you are flattening out your angle after you get flash. Also, before you insert, assess for valves and bifurcations in the vein. Try to make sure you have a nice section of vein that's straight and can accommodate the cannula, with no tell-tale lumps that are likely valves.

You're describing going through the back wall of the vein. That's why you initially get a flash but can't advance until you've pulled the needle tip back into the vein lumen. Your starting angle may be too steep or you may need to flatten it once you get a flash. Just keep at it.
