interventions are based upon the symptoms the patient is having. giving the patient's medical diagnosis is not enough information. nursing problems must be determined first. nursing problems have to do with how patients interact and cope within their environment. the way to determine nursing problems is by assessing the patient's response to their situation. this is how you will find their symptoms. then, proper and specific nursing diagnoses for that patient can be determined. nursing interventions are based upon the nursing problems and symptoms that patients have. interventions are designed to help patients improve, stabilize or support the deterioration of their nursing problem or symptoms.

What are the symptoms?

I would monitor LOC every 4 hours or qshift, monitor blood pressure, administer medications as ordered and perform neuro checks as well.
