IELTS

I have no support group to help me out im doing this application to take nclex ..I graduated from the Philippines I have a Bachelors degree in Nursing But I've been living here in the United States for almost 5 years. I got petition by my mother here in Arizona,I've been working as administrative assistant at a school at a juvenile facility for almost 4 yrs. now. My question is do I still have to take The IELTS or Toefel? Please help thank you

You need to consult with the board of nursing. Often it is based upon your country of education primary language. English is not the primary language of the Philippines so you may very well have to demonstrate English proficiency in reading, writing, speaking & comprehension as determined by the state board of nursing

According to the document linked above unless you worked in an English speaking country as a nurse for 3+ years you do need to prove English language proficiency to become licensed as an RN in AZ.
