I need help on my skill testing package + Care plans that must be done all APA style!

how in the world do i make documentation done in APA style that has a Pathophysiology and etiology for each diagnosis, disease, or problem identified in a case? and how do you make a care plan for each diagnosis, disease, or problem,

and of course, how in the world do you make Medication Cards for the drugs presented in a case???????????? especially if all you have is the name of the patitent, the age, and the diagnosis: cerebral palsy/dysphagia/ Hypertention and the medications like...Altace and lactulose?????????

i'm sooo lost... I have no idea, where to start... i don't even know how to write a etiology and sign and symptom. I have to do it for EACH DAIGNOSIS, PROBLEM , DISEASE presented >.

heeeeeelllllpppppppp meeeeeee please~ i just need someone to guide me, shine me a light how this is all suppose to look like... I don't even know how to do drug cards

Have you looked in your nursing textbook or Care plan book? ..... talked to your fellow classmates?

What information has your instructor provided to you. Have you searched our website under " Care Plans.... med cards....APA format"

Most programs follow a nursing theorist and need care plans developed based on that theory....you need to provide us with more details of what you have done so far so we can assist.

well.....what is cerebral palsy? what are the symptoms of cerebral palsy? what is dysphagia? is that from the cerebral palsy? the age is important, think about erickson's growth and development. what is hypertension? what does hypertension do to the body? what medicines would be taken for hypertension? what is the lactulose for? liver problems or gi track?

you must be first year. have you asked your classmates? do you have a care plan book?
