I need info on how to exchange my state registry from Florida to Georgia :uhoh3: can U tell me where to go or what info I can get hold of on the internet to make a copy of a form of some type to fill out and gert my registry chaned over?:uhoh21: :monkeydance: :jester:

I am a georgia NP but also hold a florida RN license. I had to apply to the state of florida for a license and have georgia send verification of licensure to the state of florida. Takes about 4-6 weeks to get done. Sounds like you are doing things the other way around, need to get forms from the georgia board of nursing website to apply for out of state licensure. There should be instructions about applying for georgia license and requirements on the website. You will probably fill out the paperwork and then request the florida department of health(nursing board) to verify you current status so that the georgia board of nursing will issue you a license.

The board of nursing is very helpful if you have trouble navigating the website.

In looking up this information for you I got a little confused because I found two very different addresses for Georgia Nurse Aide Registries. So, I am listing both addresses and phone numbers here for you. I did check both of their websites. I couldn't find any specific information about changing your certification over to Georgia on the first site, so I think your best bet is to just call their office. The second website for Georgia Health Partnership I could not get into as it is a secured site.

If you are having a problem finding it go to yahoo or google and type in georgia board of nursing. the address that i found is

the georgia board of nursing is located in macon georgia not atlanta.

ok, here's what i found.

the phone numbers for the georgia board of nursing is

the address is 237 coliseum drive

when you get to the main page go to download forms and you will see numerous forms to download. you will need to download a license by endorsement form. it will ask you all the pertinent info. regarding your initial state of licensure, etc. looks like the fee is $60.00

Thanks for your advice. I mailed a copy of my license to the Florida Department of Health with a letter stating that ask to have myself removed from the state of Flordia Registry to Georgia Registry...thats a start. I should hear from the by letter of email soon or a new card with my new address and stating that I have been transfered to the Ga. Registry.:penguin:
