I need examples of clinical maps, concept maps ect

I am on the documentation committee at my hospital and was tasked to make some care maps Clinical Pathways. I do not want to recreate the wheel.

I know there are places using them. I need some examples. Also vendor info. Is anyone using them?

Many good books on the market..have you checked your hospitals library?... if no library then try local school of nursing.
