hey guys! I need a little help. I'm working on a care plan for a 66 y/o man in a Long term Care facility. He is a very spiritual man, but unfortunately his health has declined so much that he cannot leave the facility for his church services. Although the facility offers devotional services a couple times a week, he isn't fully satisfied with this. he says "well, it's better than nothing". So far my Nursing dx is as follows: "Risk for impaired religiosity r/t LTC placement secondary to multiple disease processes m/b depression, lack of transportation et verbalization of desire to attend own church services." however, I'm stuck.. I'm not sure what to use at outcome criteria. could you help please!?!?! Thanks in advance!

I might not be much of help, as I have never heard of a person's personal activities not retaining to his illness be used as a nursing dx. I may be wrong but anywho. You could have family members tape the service for him or see if someone from the concregation can come visit, you know like his own personal service. That's all I got. Hope that helped just a lil bit.:)

the definition of risk for impaired religiosity is at risk for an impaired ability to exercise reliance on religious beliefs and/or participate in rituals of a particular faith tradition. (page 177, nanda-i nursing diagnoses: definitions & classification 2007-2008). from everything you've stated, this is already occurring. you can read about the diagnosis, outcomes and interventions on this webpage: impaired religiosity. he is impaired religiosity r/t declining physical health aeb expressions of emotional distress because of separation from own religious community and desire to connect with them.
one of the easiest ways to determine outcome criteria is to look at what your interventions would be. outcomes/goals always predict what will occur once your interventions are carried out.
one of the interventions i would do for this man is to contact the minister of his church to see if there is a way to get someone to give this man a ride to the church services every sunday. it is also possible that the minister might be willing to hold a small service once a month in the facility for him and any church members who are willing to come. at least contact the guy and see what he is willing to do for one of his members.
