I'm havein troublr coming up with nursing dx so far I have:Ineffective peripheral tissue per r/t atherscerlotic occlusion/processesRisk for infection r/t ????High risk for falls r/t ????Knowledege deficit r/t I think current health status/ disease process. But idk???Impaied short term memory r/t????Self care deficit bathing dressing/grooming/toleiting r/t???? Please help me even the slighest bit can make a huge difference. If I don't pass this I fail nursing school and I only have 9 weeks left!
Oct 4, 201114 yr I would take out the "risk for infection" since he already has one (the UTI right?). Maybe add in risk for aspiration r/t TIA or altered mental status? I'm a student too, so I might not be very helpful. Good luck!
Oct 5, 201114 yr Remember saftey + ABCs for your patient. With a stroke, there is always an increased risk for another clot - arms/legs/heart/or brain. Anticoagulation therapy, and ROM of the extremity along with neuro checks are a big one. In your assessment, look for changes in LOC. Both UTI (in the older pt) and change in NV status can lead to confusion and falls.Self care deficit: think disuse of affected hemisphereHow about aspiration? Swallow reflexes. These are suggestions, but I hope you see where they are coming from. It's up to you to piece them together. Good luck.

Oct 6, 201114 yr Author ummm actually i doknow how to do care plans im just scared and dont want to mess anything up. not evryschool has there students do care plans the same so before u judge just remeber when u went through a hard time in school and just needed a littile reassurance. if u never struggled kudos to you. however i have made it through this far because im smart.
