HELP with nursing dx!

Published

I'm working on my first careplan of the semester and I need a little help! I have a patient 9 day post appy. He is still hospitalized due to an ileus that formed after the surgery. I've done all my assessments and most of my careplan but I am stumped with my last dx. My patient was extremely anxious and "fed up" with being in the hospital. He wanted to DC all orders (NG tube, IV, labs, xray) and be discharged. Through my assessment and communication I learned that he is an active father of 5. He misses his children terribly and feels like he is "missing out" on his family. Since they live 30 mins away and the childrens school/afterschool activities and wifes work sched interfere with visiting, he only gets to see his 2 year old during the week. He also expressed concern for missing work (he works for self and is afraid of losing clients).

I can't seem to find the right dx for this situation.

I've looked at:

anxiety r/t hospital stay

Interrupted family proccess r/t patient hospitalized

I just can't seem to get this one right.

Any help/advise/suggestions are appreciated!

Thank You!

Personally I think you're stressing way too much about this. I think either diagnosis would be fine and fits the situation. The big question is what are you going to do during clinical to get this man better and home to his family:)

You don't have to have just ONE nursing diagnosis that covers everything, you know. You're giving evidence for several, looking at the list of defining characteristics and related factors. That's excellent-- so many students just pick the most obvious and never consider anything further. That minimalist approach has always seemed a little (....mmmmm, trying to think of a nice way to say "intellectually and operationally lazy" ....) to me.

If all you need to do is pick one, then just pick one. :) Or pick several, but only write up one if that's your assignment. The ones you identified would work fine. Here are two more that you can think about.

Ineffective coping

Situational low self-esteem

Good job

Putting the medical stuff aside.......I agree with what they said....;)

Clearly this guy is anxious, concerned about bills, missing his family and children....but he also has an edge. He is fearful? Is he already in financial trouble?

All of those apply
