Help IL visascreen

Could anyone kindly tell me where I should submit this form in Illinois: request for validation of registration/license for visascreen.

I called IL nursing board many times, but the lines were always busy. Could anyone provide me the information about the address and fees? Your help is highly appreciated!!!

You just need to have the licensing authority that you passed the NCLEX for complete that form and submit it to ICHP/CGFNS.

There are already several threads on this very topic, dealing with VSC and Illinois. Please just do a search for it.

hi, do the ffg. to forward the "request for verification of registration/licensure for visascreen" form to idfpr:

in order to have a certification of licensure completed and sent to another u.s. jurisdiction, you must put your request in writing to the department along with the required fee of $20, for each certification of licensure to be completed.

your request must indicate your name, profession, license number and/or social security number, and the name(s) of the licensing boards where the certification of licensure is to be sent. please make the fee payable to the department of professional regulation in the form of a check or money order.
