Help with identifying a specific NCP

My client had a carotid endarectomy done. I have been looking through ackleys and a couple other books as I would like to merge two ncps to take care of both situations at once. Not only did the client i am working on have the carotid endarectomy, he is also aphasic (warnikees? spelling). It was difficult to get information out of him.

Here are my thoughts: I need something that identifies the fact that he has been having frequent TIAs at the same time it needs to have neurovascular checks. As I learned the greatest risk of caroitd endarectomy is having a clot. When i looked for information in my books it basis information on care giver strain, ambulation, self-care def. etc..

If anyone has any ideas, or some idea for me to look elsewhere other then in cardiac care plans please let me know.
