Help with Care plan!

I am in my last 3 months of LPN school and am stuck on 2 dx's for pt with CVA...hx - 65 y/o female, ischemic stroke, right side hemiparesis, dm, retinopathy, MI, ....my mind is at a complete blank! I need something where I am able to perform and measure the interventions during my clinical time...ugh!!!!!!!!!! 3 more months to go!!!!!!!!!!! ;-)

a care plan starts by listing the signs and symptoms that a patient has. the only symptom you mention is right side hemiparesis. you might want to read this tutorial about strokes: http://www.strokecenter.org/prof/ems/ - stroke interactive tutorial to learn about the signs and symptoms of a stroke. nursing diagnoses and then nursing interventions are developed from those signs and symptoms.
