I know this has been asked 1000 times, but having a hard time with head to toe assessments. I got a bunch of questions. What is your best reference book for head to toe assessments? Anyone know anything with pictures (real pictures not the made visual/made easy cartoony pictures). I need to see what things look like in real life. Also any resources that help with documentation. I'm struggling with narrative charting on my head to toes. Oh also in real life do most head to toe assessments include PERRLA? I've never seen an actual nurse do this so wondering if this is usually included in head to toes or just something you use in nursing school.
