Was hoping for some help with understanding the below answers:

Question: You gather the following assessment data, which of the following cues form a pattern? (Choose all that apply)

1. Client is restless

2. Fluid intake for 8 hours is 800 mL

3. Client states feels short of breath.

4. Client has drainage from surgical wound.

5. Respirations are 24 per minute and irregular.

6. Client reports loss of appetite for over 2 weeks.

Answers: 1,3, 5

I answered 2, 6

I totally missed it. Any help would be great.:confused:

What is the question asking?

It is asking for the responses that form a pattern.

What pattern does 2 & 6 form?

What pattern does 1,3,5 form?

I see that there is suppose to be a pattern, but

for exampe, a client being restless is not a pattern it's just a single datum.

Client feels short of breath looks like a single datum also, I do not see a pattern unless client feels short of breath over a week, or some period of time to show a pattern.

Respirations are 24 beats per minute and irregular also is a single datum. I don't see a pattern with a single fact unto it self.

Please elaborate on the patterns.

You're right. It's a single data point, but all data points put together form patterns.

Sometimes it helps to see patterns by putting yourself in the situation. If you're short of breath, what are you also feeling? You might feel restless. What are you also doing? You're breathing faster.

Think about pain. You can't feel someone else's pain, so as a nurse, what do you see that prompts you to ask someone about their pain? Facial grimacing, guarding, muscle tension, favoring a certain side of the body, etc. Each of those are single data points that, when taken together, form a pattern that would prompt you to ask the patient for a pain rating.

I think that the common thread of 1, 3 and 5 is the fact that the patient is starting to show signs of hypoxia. Since restlessness is one of the first signs of hypoxia, that is what I would have based this question off of.
