Frequency calendar (10 weeks) to map visit dates

I am looking for some type of program that I can use to print a 10 week calendar for my patient. I have heard there is one that you can type in the SOC and the program would then print a 10 week calendar from that date. Anyone out there know about one? Currently, we have a "generic" 10 week with empty boxes. We then have to write in the dates for the whole 60 day episode. This is such a mundane thing that takes so much time.:uhoh3:

I have one in an excel format that you can type in the dates if you want, but no program that will generate it. I'm happy to email the excel file to anyone who will PM or email me their email address.
