Form 1 help!!!! Retake of NCLEX

I have a question... when applying to retake the NCLEX. I have been told to fill in FORM 1 which is on the NY state boards web site.. item 22 wants evidence of Chlid Abuse Identification... do I need to go and get certified again since the last time I applied to take NCLEX was ten years ago....(its a long story)

Yes you will have to do the Child abuse again, Think you have to do it every time you renew license also
