Perimenopause. Dunno if that's true, but dayum it's messing me up 

Well, we are Octopus, let's not forget that.  Are you overwhelmed with home life or something else, it could be that.  Either way, don't beat yourself up, slow down and keep focus.  It will be fine.

There are several things that could have contributed; construction on the unit, this particular patient was getting three different antibiotics, I work nights...I know mistakes happen and 100% own up to the mistakes. I filled out an incident report on myself. It doesn't help my self-esteem when the supervisor for the unit is toxic. Never says anything positive, less than helpful, demeaning, etc. it's night shifts fault, all the mistakes happen on night shift. Basically we suck. 

There have been other things, like forgetting how old my only child is - I had to ask him, it took me 4 days to realize I was washing my hair with conditioner, got home from work and forgot I didn't have house keys. I sent a message to my PCP asking to have my hormone levels checked as I have an IUD so missing periods is 'normal' for me.

Hmm, this is interesting.  Change of thought, I think this is worth bringing up to your PCP.
