Do the staff nurses for Rehab also do the fim scoring? We chart what the patient does as far as bathing eating, dressing, toileting ect... Share with me who submit your scorings.

I work at a for-profit rehab hospital. Staff nurses do an initial FIM score upon admission, then once every shift for the first 72 hours of the inpatient stay, then a revised score is done upon discharge. The rehab department also does their own FIM score.

We do fim scores the first 3 days of admission, and the day before they leave (24-hour period); nurses do a part, OT/PT and Speech do their parts, and someone else combines all of it to actually submit these.

Nope. We know what they are (just took a test on that) and can get to a number if i have to, but the only input we are asked for is bowel/bladder. The other disciplines do their area of expertise. Some of the staff (PT/OT/RT) use numbers when describing, some use the words.

Thank you, we fill out forms for the first 3 days and then the last before discharge, somehow lately we have been filling out these lenghty forms for every shift, it's very time consuming. Thank you for your responses.
