Wondering if anyone would share what your facility is using for a Fall Risk Assessment tool. The one we are still using is based on the 2.0 MDS, and of course we need one based on the 3.0. Thank you very much.

We still have the 2.0 based one; I've been trying to revamp the assessments, but so far I haven't found one I liked.

Your risk assessments should be based in reality not on the MDS 2 3 4 5 or whatever. Briggs makes a good falls risk assessment form. The MOST important part of doing the assessment, is making a plan to try to mitigate any possible risk.

please google national patient safety and also enter Morse scale... you should be able to get the website that includes the protocols that goes with it and the assessment also includes intrinsic and extrinsic factors which is answers the information on the CAA part of the MDS.

One that I've come up with, that is in similar format of our prior fall assessment - if anyone wants to see it, I can email it to you. The site won't let me upload it for some reason.
